Instructions for Submitting College Applications
Obtaining College Application Forms:
Most colleges prefer the on-line registration process. However, paper applications can be obtained from the college website or in the EVHS Career Center in the Guidance Office.
Applications fees must accompany the paper application form or online registration. Please refer to each college application guideline for cost of the application fee and type of payment accepted.
High School Counselor Report Form:
The High School/Counselor Report Form may either be included in the paper application or a separate attachment. When required, students registering online will download the High School/Counselor Report Form. Please complete the applicant portion of the report, sign and submit your application to the guidance office.
Submitting Final Applications to the Guidance Office:
Students submitting applications to the guidance office for processing must complete and attach the College Application Routing Form to the application. Students must indicate the following: name, student #, counselor name, college/university name or scholarship, application deadline, type of transcript (regular or honors) and which ACT, SAT test score to report.
The counselor will complete all necessary paperwork and mail it to the respective college, along with a senior year schedule and all other submitted documents.
During peak periods, college applications can take more than 10 school days to process. Students are responsible for knowing deadlines and bringing application materials to the guidance office in time for proper processing.
Ordering High School Transcripts:
EVHS Transcripts should be ordered electronically through Docufide, a secure transcript provider. Students can access this service through the EVHS website and click on the Docufide icon located on the home page. There is a $5.00 fee for each transcript processed.
Requesting Letters of Recommendations from EVHS staff:
Not all colleges require a Letters of Recommendation (check application). If required, students can obtain a packet in the guidance office that outlines the protocol for requesting letters of recommendations from EVHS staff members. This packet can also be obtained from the EVHS website under student services/guidance office/college preparation. Each student is responsible for coordinating and verifying the delivery of these letters from the recommending teacher.
Submitting Mid Year Reports:
College Mid-Year Report forms, if required, should be submitted to Pat Gerrits in the guidance office before the end of the first semester. Counselors will complete and mail these reports to the respective colleges along with 1 st semester transcripts.
Please do not order official transcripts from Docufide for Mid Year reports. There will be no charge for these transcripts.
Reporting Test Scores to Colleges:
Eastview High School does not release student test scores to colleges, NCAA or scholarship agencies. It is each student's responsibility to release official ACT or SAT test scores to these agencies. If a student needs to order additional score reports, or if score reports were not requested for colleges at the time of registration, they can be ordered online at www.act.org or www.collegeboard.com or by mail. These requests are processed after your test has been scored and results have been mailed. |
